Ms. Artist Passport released

Last November 5, 2021, Ms. S (privacy respected) personally received her 15 year old daughters*s passport. Photo is at the Food Court of the mall.

Her daughter is an avid and talented artist and I watched her drawing with awe while waiting for her cue during her personal appearance at the DFA.

Most kids surf thru the net using their gadgets, her daughter draw. And she loves to draw anime characters. I will ask if she could share one with us one of her works.

We had to resolve the prerequisites by the mother, in the meantime assisting her teen age girl.

So the processing to DFA was hitting 2 birds with one stone.

Here, while mother submits her finally released PSA birth certificate , which was NSO certified but was not uploaded into the PSA system, upon which we had to work on for the local civil registrar where she was born will send it to PSA Baguio for uploading, she also received the copy of her kids passport at the age of 15.

Take note that minors* passports are good only for five (5) years because they change and outgrow what they used to look like.

Information to be provided for the issuance of Birth Certificate:

1.     Complete name of the child (first, middle, last)
2.     Complete name of the father
3.     Complete maiden name of the mother
4.     Date of birth (month, day, year)
5.     Place of birth (city/municipality, province)
6.     Whether or not registered late. (If registered late, please state the year when it was registered.
7.     Complete name and address of the requesting party
8.     Relationship to the child
9.     Number of copies needed
10.     Purpose of the certification

NOTE: ISSUANCE OF CERTIFICATION OF BIRTH IS SUBJECT TO THE CONFIDENTIALITY CLAUSE OF ARTICLE 7 OF THE CHILD AND YOUTH WELFARE CODE WHICH PROVIDES:

Art. 7. Nondisclosure of birth records. The records of a person's birth shall be kept strictly confidential and no information relating thereto shall be issued except on the request of any of the following:
 

1. The person himself, or any person authorized by him;
2. His spouse, his parent or parents, his direct descendants, or the guardian or institution legally incharge of him, if he is a minor;
3. The court or proper public official whenever absolutely necessary in administrative, judicial or other official proceedings to determine the identity of the child's parents or other circumstances surrounding his birth; and
4. In case of the person's death, the nearest of kin.

Any person violating the prohibition shall suffer the penalty of imprisonment of at least two months or a fine in an amount not exceeding five hundred pesos, or both, in the discretion of the court.

Requirements for Courtesy Lane Eligibility

 The following may avail of the Courtesy Lane Facility:

1. Senior citizens
   • A senior citizen applicant may be accompanied by one (1) adult. The adult companion may also avail of Courtesy Lane privileges, provided they are the senior citizen applicant’s immediate family member or traveling companion.
   • An immediate family member (i.e., spouse, adult children, or adult sibling only) must present proof of relationship to the senior citizen applicant (e.g., birth certificate).
   • A traveling companion must present proof of travel with the senior citizen (e.g., confirmed flight or hotel booking) and must also state the reason or justification for travelling (e.g., the travelling companion is a medical escort or nurse).
2. Persons with disability (PWDs)
   • A PWD applicant must present a valid PWD Identification Card.
   • A PWD applicant may be accompanied by one (1) adult. The adult companion may also avail of Courtesy Lane privileges, provided they are the PWD applicant’s immediate family member or traveling companion.
   • An immediate family member (i.e., parent, spouse, adult children, or adult sibling only) must present proof of relationship to the PWD applicant (e.g., birth certificate).
   • A traveling companion must present proof of travel with the PWD applicant (e.g., confirmed flight or hotel booking) and must also state the reason or justification for travelling (e.g., the travelling companion is a medical escort or nurse).
3. Pregnant women
   • A pregnant applicant must present medical certificate/records as proof of pregnancy.
4. Minors aged seven (7) years and below
   • A minor applicant must be accompanied by his or her parents, legal guardian, or their authorized representative. The adult companion may also be an applicant.
5. Solo Parents
   • A solo parent applicant must present a valid Solo Parent Identification Card.
6. Overseas Filipino Workers (OFWs)
   • An OFW applicant must present proof of their OFW status, such as:
     • Valid OWWA E-card;
     • Valid employment contract;
     • Valid work visa;
     • Seafarer’s Identification and Record Book (SIRB) stamped at an international border not more than three hundred sixty-four (364) days from the date of application;
     • Expired or cancelled working visa or employment contract, and a Philippine passport with an immigration arrival stamp of not more than three hundred sixty-four (364) days from the date of application.
7. Exceptional and emergency cases
   • In exceptional and emergency cases, Courtesy Lane privileges may be extended to applicants upon due approval of his or her written request by any of the following Department officials:
     • The Secretary of Foreign Affairs;
     • The Undersecretary for Civilian Security and Consular Concerns; or
     • The Assistant Secretary for Consular Affairs.

NOTE

• Senior citizens and PWD applicants may choose either regular or expedited processing of their passport application during regular hours from Monday to Friday
• All other applicants including companions must pay for expedited processing.
• All applications outside regular hours must pay for expedited processing.
• Consular Offices are authorized to set their own daily maximum number of applicants based on operating capacity of the CO and the safety and convenience of the public.
• Access to the Courtesy Lane may be restricted in emergency or extraordinary circumstances such as in the present Covid-19 public health emergency.

Supporting Documents for Adult New Applications

In addition to the core requirements, the following supporting documents will be further required in the following cases:

In case of Late Registered Birth Certificate

• If Birth Certificate was registered at least ten (10) years ago, application will be treated as a regular application, and no additional supporting documents will be required
• If Birth Certificate was registered less than ten (10) years ago, applicant must submit IDs that pre-date the late registration or current IDs with NBI Clearance

In case applicant has NO Birth Certificate or Report of Birth:

• If applicant is born AFTER 1950 - applicant must first file for late registration with Local Civil Registrar (LCR) or Consular Office with jurisdiction over the place where applicant was born. Applicant will then submit the original copy of the PSA authenticated late registered Birth Certificate and IDs that pre-date the late registration or current IDs with NBI Clearance.
• If applicant is born ON OR BEFORE 1950 - applicant must submit original and photocopy of PSA Authenticated Certificate of No Birth Record and Affidavit of Two Disinterested Persons attesting to the identity of the applicant (Affidavit should include photocopy of IDs of the Two Disinterested Persons indicated in the presented document)

If an Applicant's marriage has been dissolved and wants to revert to her maiden surname

• Applicant must submit original and photocopy of PSA Birth Certificate and acceptable IDs.
• Applicant must submit original and photocopy of the annotated PSA Marriage Certificate (MC) or Report of Marriage (ROM) stating that the marriage has been dissolved; OR

If applicant has lacking data in Birth Certificate or Report of Birth

• Applicant must present original and submit photocopy of proof of filing of supplemental report from the Local Civil Registrar (LCR)

If applicant has discrepancy in data in Birth Certificate / Report of Birth and other documents

• If the discrepant data is in the BC, the applicant must submit annotated birth certificate authenticated by PSA reflecting the corrected entry. If the discrepant data is in the other documents, the BC will be followed.
• Discrepancy in first name - Applicant must submit the original and photocopy of the petition and supporting documents for correction of first name filed with Local Civil Registrar (LCR) or consulate
• Discrepancy in other data - Applicant must submit annotated birth certificate authenticated by PSA reflecting the corrected entry

If applicant is a Dual Citizen

 List of Acceptable IDs for Philippine Passport Application (Any of the following with one (1) photocopy):

• Social Security System (SSS) Card
• Government Service Insurance System (GSIS) Card
• Unified Multi-Purpose Identification (UMID) Card
• Land Transportation Office (LTO) Driver’s License. Student Permit may be accepted if in card format. 
• Professional Regulatory Commission (PRC) ID
• Philippine Identification (PhilID)
• Overseas Workers Welfare Administration (OWWA) E-Card
• Commission on Elections (COMELEC) Voter's ID or Voter's Certificate issued from COMELEC main office in Intramuros, Manila (Certificates issued in COMELEC's regional offices may require further verification. Applicants are requested to present NBI or police clearance along with the Voter's Certificate).
• Philippine National Police (PNP) Permit to Carry Firearms Outside Residence
• Senior Citizen ID
• Airman License (issued August 2016 onwards)
