List of Acceptable IDs for Philippine Passport Application

 List of Acceptable IDs for Philippine Passport Application (Any of the following with one (1) photocopy):

  • Social Security System (SSS) Card
  • Government Service Insurance System (GSIS) Card
  • Unified Multi-Purpose Identification (UMID) Card
  • Land Transportation Office (LTO) Driver’s License. Student Permit may be accepted if in card format. 
  • Professional Regulatory Commission (PRC) ID
  • Philippine Identification (PhilID)
  • Overseas Workers Welfare Administration (OWWA) E-Card
  • Commission on Elections (COMELEC) Voter's ID or Voter's Certificate issued from COMELEC main office in Intramuros, Manila (Certificates issued in COMELEC's regional offices may require further verification. Applicants are requested to present NBI or police clearance along with the Voter's Certificate).
  • Philippine National Police (PNP) Permit to Carry Firearms Outside Residence
  • Senior Citizen ID
  • Airman License (issued August 2016 onwards)
  • Philippine Postal ID (issued November 2016 onwards)
  • Seafarer's Record Book (SRB)(*must be issued Feb 2020 onwards) issued by the Maritime Industry Authority (MARINA)
  • Valid or Latest Passport (For Renewal of Passport)
  • For minor applicants, School ID OR Certificate of Enrolment with photo of minor and dry seal of school may be presented

Note

  • For applicants based overseas, they may use their host government issued IDs showing their Philippine citizenship. (Example: Residence Card)
  • All IDs presented should be readable, untampered and contains consistent information with the documents presented upon application.
  • Original and photocopy of IDs and supporting documents are required to be presented upon application.
  • Important Reminder: The Department may require additional supporting documents as may be necessary.

Core Requirements for Adult New Applications

  1. Confirmed Online Appointment *eto po yong binabantayan natin online. 
  2. Personal Appearance
  3. Accomplished Application Form
  4. Original and photocopy of Philippine Statistics Authority (PSA) Authenticated Birth Certificate on Security Paper
    • Married Females (who are using their spouse's last name) must also present Original and submit photocopy of PSA Authenticated Marriage Contract on Security Paper or Report of Marriage
    • Local Civil Registrar Copy is required if PSA Birth Certificate is not clear or cannot be read
  5. Any of the following acceptable IDs with one (1) photocopy. List next article. or in the page of DFA, just click the link. 

NOTE: 

If woman opts to retain maiden name, a Marriage Contract is not required.

On Passport Online Appointment Booking Assistance

True, the government created has created means and ways to give its passport applicants an easier way to book for an appointment with the Department of Foreign Affairs whereby an applicant can book themselves online which is the following at its website https://www.passport.gov.ph/  and they also removed the accreditation to do business given to travel agents for a weekly slots intended for its clients. 

For some time, we have even been scared assisting people because we were warned and harassed by people because my kaapelyido warned us that we are not allowed to process passports for our clients. 

Of  course we do not process the passports. We only assist our clients prepare and submit the and follow up the processes  of the needed papers so that the applicant will comply with the requirements. We assist our applicants book an appointment. 

However, not everyone are wired or online and that many Filipino citizens despite having smart phones and having mobile data, most government websites are so heavy they cannot read information that they need to know. 

What is the option? Ask somebody to give them information by manual communication and in person. And when they have a problem with their papers, they cannot go to any DFA office just to inquire and they do not have an access to telephones.  But even if we have telephone, calling up takes several days before we are answered. And if they will talk, the problem is what questions to ask the person on the phone. 

So I am justifying the services that people like me provide. 

Our fellow either do not have access nor the time to stay online almost 24 hours a day just to GRAB an appointment with DFA. For this they hire our services and we are glad to work and assist them. 

Random screen shots show the distance and days whereby we work our very best to catch an appointment. It took me more than 2 months just to catch appointments due to the ECQ and the GCQ and the glitches of the websites of DFA. Other monitoring were not screened shot but we keep refreshing the site at least every 15 minutes 24 hours a day using either a cellphone or the laptop or others.
